What is the Long-Term Youth Center?

The Long-Term Youth Center (LTC) specializes in  Residential Treatment Centers for pre-adolescent and adolescent males and females. Developed to provide a safe and secure environment to foster recovery and a return to a higher level of social function for those individuals who need Residential Treatment, the Long-Term Youth Center at Mountain View Hospital works with proven methods of counseling and medication management to help ensure a positive result.

What is the Treatment Team?

The Long-Term Youth Center focuses on the behavioral, familial and educational aspects of care for the individual. This is accomplished by developing individualized treatment plans that stress individual strength and in turn, meet the needs of the individual. The Treatment Team includes:

Physicians & Psychiatrists
Registered Nurses
Masters-Level Counselors
Rehabilitation Therapists / Adventure Based Counseling
Psychiatric Technicians
Social / Counselors
Special Needs Educators
Psychologists

Each member of the Treatment Team works together daily to ensure that the individual’s needs are being consistently addressed. Additionally, the Treatment Team coordinates care with other necessary health care professionals and referral sources to make sure that information is properly provided.

How does the Continuum of Care—Discharge Planning Work?

Discharge planning begins from the first day. Care is taken to ensure that coordination with the Individualized Service Plan (ISP) is ensured between agencies involved. Residents are assessed on an on-going basis to determine if their needs can be met in a less restrictive setting such as therapeutic foster care, group home placement or return to the biological family.  Mountain View Family Services staff work directly with the Long-Term Care Treatment Team in locating an appropriate resource if therapeutic foster care is indicated by the treatment needs of the individual.  Referrals are made to other appropriate community resources if indicated and requested by the referral source throughout the area.
