POST DISCHARGE CONTINUUM OF CARE

Client Assessment Specialists monitor the client's improvement throughout treatment and upon discharge on an ongoing basis.  Post-discharge monitoring is determined on a case-by-case basis and may include weekly contact, support group involvement and written documentation of client progress.  The Client Assessment Manager understands that by working with referring sources the client receives the highest quality of care.  In facilitating the client's continued care, the Client Assessment Specialist is responsible for seeing that the client returns to the referring source.  Should the client require additional services, the Case Manager will remain informed about the client's progress and issues.
