INITIAL
ASSESSMENT Assessments
can be provided in the client's home. This
allows for better accessibility and promotes a more relaxed assessment process.
On-site assessments can be scheduled within a 24 hour period. REFERRAL Each
Client Assessment Specialist has the capability of referring clients to specific services
according to their needs such as outpatient counseling, services provided by
public agencies, judicial involvement, partial hospitalization, support groups
and, when appropriate, hospitalization. POST
DISCHARGE CONTINUUM OF CARE Client
Assessment Specialists monitor the client's improvement throughout treatment and upon
discharge on an ongoing basis. Post-discharge
monitoring is determined on a case-by-case basis and may include weekly contact,
support group involvement and written documentation of client progress.
The Client Assessment Manager understands that by working with referring sources the
client receives the highest quality of care.
In facilitating the client's continued care, the Client Assessment
Specialist is
responsible for seeing that the client returns to the referring source.
Should the client require additional services, the Case Manager will
remain informed about the client's progress and issues. AFTERCARE
SUPPORT SYSTEM All
clients are given the opportunity to access aftercare support groups and are
encouraged to do so. Mountain View
Hospital provides groups in specific outlying areas. Groups are available for children, adolescents, and adults. CRISIS
INTERVENTION Mountain View Hospital
understands the need may arise to access Mental Health or Chemical Dependency
services any hour of the day. By

available to assist you in dealing with crisis situations or community resource
